The registration process will require you to sign in and create an account for the parent and any children (”participants”). Once you select the “Register” button in the upper left of the screen, you will be asked to provide basic contact information for the parent and any back up contact. You will then select “Add a participant” and enter the data related to your child including date of birth and uniform sizes. If you have more than one child playing, just repeat the process.
Once you have completed responding to the questions for each child, you will see the final “order” information. If it is correct, proceed to next step. If it is incorrect, you will be able to change it and then proceed.
The last screen will be your account summary showing the complete order. You can sign off at that time and you will receive two confirming emails. First, an email with your account id and password, and the second an email to confirm your order and the required next steps. Attached to that email will be the required US Club Soccer Medical Release Form. As inidcated below, this form must be completed as part of the registration process for each child.
At this point you will have set up your player's registration. As described on the order confirmation email, to complete the registration process for any player who did not participate in YDP this past, you will need to mail in:
- A copy of each child's birth certificate
- A completed US Club Soccer Medical Release form for each child
(blank form attached to Order Confirmation email)
- A recent passport sized photo showing their face
- A check payable to MTSC - YDP for the total amount of the “order.”
If your child played in the YDP program in a prior season, requirements differ slightly. For a new fall season, we need a new medical release, photo and payment (no birth certificate). If registering for a spring season, if you child played in the prior fall we will only need the check in the correct amount. If registering for the spring and your child did not play the prior fall but played in prior years, we need a medical release, photo and payment.
