Use the Program Introduction letter & Donation Submission form to help you solicit donations for your WYS ODP program fees and participation costs.
Fill in your contact information, at the bottom of the form, and sign your name.
You must have Adobe Reader 7 and up to open and fill in the form. Go to Adobe.com to download the most current version of Adobe Reader. You can also email the state office to request these forms be emailed to you - wssa@wyomingsoccer.com
