Items to be turned in to team managers on or before your teams signing date:

1. City of Alpharetta Activity Registration Form and payment (cash, check, CC) to ARPD
2. Copy of Birth Certificate
3. Medical Release form, signed and notarized
4. Passport size photo
5. Annual Ambush Club Fee ($125) Check or Cash only. Checks payable to Ambush Soccer Club
6. Team Camp fee of $85 for U9-U12 teams, $110 for U13-U19. Cash or Checks only payable to the trainer.
7. Uniform size and 3 preferred number choices
