 Embed Google Forms on Your Site

Have you ever wanted to collect information from your parents or players and you forgot to put it in your original registration information or maybe it is halfway through the season and there is a separate form that you forgot that needs to be filled out by your parents or players. With Google Forms, you can create a document and receive results from users that fill out the form.
In order to create a form, you have to have a Google email account. In your email, you will see a tab at the top called documents. Click the create tab and create a new form.

You can then begin creating your form by asking any number of questions. Google Docs has a great help section to walk you through the process of creating a form. Once the form has been created, you can scroll under the Form tab, and select the Embed form in a webpage, which will give you a code.

Copy the code that the form gives you. Go to your website and go to the page that you want to add the form to. Add an HTML module on the page. Click on Edit Content for the module. You are going to want to select the Basic Text Box radio button at the top. Also, select Raw as the render mode at the bottom. Paste your code in and click on Save.

The form will now be available on your site. Now it is up to you to direct your users to the form. Once they fill out the form, results will be displayed to you in a form of a Google Doc spreadsheet.
